Kate Middleton’s recent cancer diagnosis has highlighted the importance of early detection to us all. Detecting potential health issues early means individuals can work with healthcare professionals to implement treatment plans, lifestyle changes, or preventative measures. An article published by the Daily Mail, which focuses on early cancer detection MOT’s, emphasises the importance of preventative testing and treatment throughout. Biomedical Science Day celebrates the vital role biomedical scientists play in patient care through the diagnosis of infections and diseases. World Heart Day – Raising awareness of Cardiovascular Diseases

On 29th September, World Heart Day is an opportunity for everyone to stop and consider how best to use heart for humanity, for nature, and for yourself. Beating cardiovascular disease is something that matters to every beating heart.

In May 2012, world leaders committed to reducing global mortality from non-communicable diseases (NCDs) by 25% by 2025. Cardiovascular disease (CVD) is accountable for nearly half of all NCD deaths making it the world’s number one killer. World Heart Day is, therefore, the perfect platform for the CVD community to unite in the fight against CVD and reduce the global disease burden.

World Heart Day is a global campaign created by the World Heart Federation in which it informs people around the globe that CVD, including heart disease and stroke, is the world’s leading cause of death claiming 18.6 million lives each year. It aims to highlight the actions that individuals can take to prevent and control CVD as well as to drive action to educate people by controlling risk factors such as tobacco use, unhealthy diet, and physical inactivity.

What are cardiovascular diseases?

Cardiovascular diseases (CVDs) are a group of disorders relating to the heart and blood vessels and they include:

  • coronary heart disease – disease of the blood vessels supplying the heart muscle
  • cerebrovascular disease – disease of the blood vessels supplying the brain
  • peripheral arterial disease – disease of blood vessels supplying the arms and legs
  • rheumatic heart disease – damage to the heart muscle and heart valves from rheumatic fever, caused by streptococcal bacteria
  • congenital heart disease – malformations of heart structure existing at birth
  • deep vein thrombosis and pulmonary embolism – blood clots in the leg veins, which can dislodge and move to the heart and lungs

Heart Health Test

High Cholesterol is a major risk factor for heart disease and stroke therefore at Randox Health we have the Heart Health Test to keep our cholesterol at a healthy level. This is done from the convenience of your home with our home sample collection kit. Heart Health measures your total cholesterol, HDL (good) cholesterol, LDL (bad cholesterol) and triglycerides.

World Heart Day was established by the World Heart Federation in collaboration with the World Health Organization to raise awareness of cardiovascular diseases.

Cardiovascular disease (CVD) is the world’s number one killer, causing over 18.6 million deaths per year, according to the World Heart Federation. 85% of these deaths are due to coronary heart diseases (e.g. heart attacks) and cerebrovascular diseases (e.g. strokes) and mostly affect low-and middle-income countries.

Your heart is amazing. Not only is it your most critical organ but also one of the most hard-working. The average adult heart beats around 100,000 times a day, acting as a giant pump for all the blood in your body. Indeed, every day your heart pumps over nine litres of blood through a system of blood vessels over 60,000 miles long – it’s little wonder, then, the importance placed on looking after such a vital muscle.

Did you know?

Cardiovascular Diseases (CVDs) are the number 1 cause of death globally, more people die annually from CVDs than any other cause.

 

Over time, and influenced by lifestyle choices, the heart grows weaker, needing to work harder to fulfil its function. Crucial lifestyle changes now could limit your risk of developing serious cardiac conditions, such as Cardiovascular Disease (CVD) in the future.

While lifestyle changes may help to limit your risk of CVD, and related heart condition, it is impossible to eradicate it completely for everyone. Cardiovascular disease is the number one cause of death globally. Across the world, 17.7 million people died from CVDs in 2015. In the UK, it accounts for nearly one death around every eight minutes.

But early screening and improved access to cutting-edge diagnostics could lower this figure significantly. According to the WHO there are 8 key risk factors of developing CVD and they account for as much as 61% of all cardiovascular deaths, these include;

Smoking

The harmful substances in tobacco can damage and narrow blood vessels, causing CVDs. Smoking kills approximately half of persistent smokers, around 14% of deaths from circulatory disease can be attributed to smoking.

High Blood Pressure

High blood pressure (hypertension) is one of the most important risk factors of CVD, if your blood pressure is too high your blood vessels can become damaged.

High BMI

Being overweight or obese increases the risk of developing diabetes and high blood pressure, both of which can cause CVD. You’re at an increased risk of CVD if your BMI is 25 or above.

High Cholesterol

Cholesterol is a fatty substance found in the blood, high levels of cholesterol can cause blood vessels to narrow and increase the risk of developing a blood clot. Reducing the populations LDL-Cholesterol by 5% would prevent 64,000 cases of CVD.

Lack of Physical Activity

If you don’t exercise regularly, it’s more likely you’ll develop high blood pressure, high cholesterol levels and become overweight. All of which are risk factors of CVD. Exercising regularly helps to keep the heart healthy.

Other CVD risk factors include an excessive alcohol intake, an unhealthy diet and high levels of blood glucose.

Cardiac health and regular cardiovascular screening is important to enable risk factors to be detected in their earliest stages. sLDL cholesterol

sLDL cholesterol  is a vital cardiac risk marker which utilises the “Denka Seiken” method which produces results in less than ten minutes. Research has shown individuals with a predominance of sLDL have a 3-fold increased risk of myocardial infarction (MI), even for those who HDL, LDL, Total Cholesterol and Triglyceride levels are considered ‘normal’. Elevated levels of sLDL are associated with a sedentary lifestyle, a diet high in saturated fat, insulin resistance, pre-diabetes and genetic disposition. Measurement of sLDL allows the clinician to get a more comprehensive picture of lipid risk factors and tailor treatment accordingly. Reducing LDL levels reduce the risk of both CVD and MI.

Lipoprotein (a)

Lipoprotein (a) is a major independent genetic risk factor for premature CVD and elevated levels can occur in patients with otherwise normal lipid levels, therefore providing much needed information in relation to a patient’s risk of developing CVD.Along with other tests, Lp(a) can provide additional information on a patient’s risk factor of developing cardiovascular disease (CVD).

The Randox Lp(a) assay is one of the only methodologies on the market that detects the non-variable part of the Lp(a) molecule and therefore suffers minimal size related bias providing more accurate and consistent results.

HDL3 Cholesterol

HDL3 Cholesterol is a smaller and more dense sub fraction of the HDL particle. HDL comprises of several subclass particles, which differ in their sizes, densities and components. HDL is the scavenger of cholesterol within arterial walls and the levels of HDL3 is too low, the ability to remove this cholesterol is reduced. Therefore, it is widely accepted that there is an inverse correlation between HDL3 and cardiovascular disease risk.

Acusera Infectious Disease Serology Controls

There is approximately 1 million people in the USA living with HIV and around 15% of them are unaware they are infected (CDC, March 2019). Furthermore, the Centers for Disease Control and Prevention (CDC) estimate that around 39,000 new HIV infections are reported per year.

The impact of not only HIV but all infectious diseases is getting much greater. Therefore, laboratories now play a much more important role in providing accurate test results. This is a key component in controlling the spread of infections.

Lipoprotein(a) [Lp(a)]

 Lipoprotein(a) [Lp(a)] is described as “the most complex and polymorphic of the lipoprotein particles” 1. The biggest challenge that exists surrounding Lp(a) measurement is the heterogeneity of the apo(a) isoforms. Standardisation across calibrators is vital as the repeated KIV-2 units in Lp(a) act as multiple epitopes. A lack of standardisation could result in the underestimation of Lp(a) associated CVD risk 2.

Lp(a) assays that are standardised to the WHO/IFCC reference material, transferring values from mg/dl to nmol/l are more uniformed. The Denka Seiken Lp(a) assay is the most reliable commercially available Lp(a) assay, because 2:

  1. The isoform size variations are reduced as a range of calibrators from separate pools of serum were used which covered a range of Lp(a) concentrations.
  2. The isoform size and concentrations are inversely correlated better matching calibrants with test samples.
  3. Methods based on the Denka Lp(a) assay, calibrated in nmol/l and traceable to the WHO/IFCC reference material produced an acceptable bias compared with the Northwest Lipid Metabolism and Diabetes Research Laboratory (NLMDRKL) gold standard method.

 

HEART UK advocate the widespread clinical utility of Lp(a) to refine the CVD risk assessment. In doing so, HEART UK developed ten statements on Lp(a) (Table 1) utilising the 2016 ACC/AHA Clinical Guideline Recommendation Classification System, to assess the strength of the recommendation and aid in guiding the phrasing of the recommendation 2.

H-FABP in CSA-AKI Testing

Cardiac surgery-associated acute kidney injury (CSA-AKI) is a well-recognised postoperative complication of cardiac surgery and is the second most common cause of AKI in the ICU, occurring in up to 30% of patients. Higher pre-operative values of H-FABP were associated with a higher risk of developing AKI. Following adjustments of certain patient characteristics, the association of pre-operative H-FABP with the development of AKI remained significant. In fact, the researchers observed that a 1 unit increase in log H-FABP was associated with a 3-fold increase in the odds of developing AKI.

There is approximately 1 million people in the USA living with HIV and around 15% of them are unaware they are infected (CDC, March 2019). Furthermore, the Centers for Disease Control and Prevention (CDC) estimate that around 39,000 new HIV infections are reported per year.

The impact of not only HIV but all infectious diseases is getting much greater. Therefore, laboratories now play a much more important role in providing accurate test results. This is a key component in controlling the spread of infections.

Reporting a false positive result has its own obvious implications, however, what about the consequences of reporting a false negative. This can be just as devastating to a person in terms of the treatment they will receive and even personal lifestyle changes for the patient in question.

Adiponectin

Adiponectin (adipocyte complement-related protein of 30kDa (Acrp30)) is an adipokine (protein hormone) produced and secreted by the adipose tissue, an endocrine organ. Adiponectin acts as a messenger in the communication of adipose tissue and metabolic organs. In doing so, Adiponectin suppresses the production of glucose in the liver through inhibiting the genes involved in glucose production and enhances fatty acid oxidation in skeletal muscle 2. Consequently, Adiponectin is a strong protector against several pathological events in various cells through inhibiting inflammation, suppressing cell death and enhancing cell survival. Adiponectin has been identified as having pleiotropic functions widely associated with anti-atherogenic, anti-diabetic, cardioprotective and anti-inflammatory effects.

H-FABP in CSA-AKI Testing

Cardiac surgery-associated acute kidney injury (CSA-AKI) is a well-recognised postoperative complication of cardiac surgery and is the second most common cause of AKI in the ICU, occurring in up to 30% of patients. Higher pre-operative values of H-FABP were associated with a higher risk of developing AKI. Following adjustments of certain patient characteristics, the association of pre-operative H-FABP with the development of AKI remained significant. In fact, the researchers observed that a 1 unit increase in log H-FABP was associated with a 3-fold increase in the odds of developing AKI.

Randox Laboratories has this month announced its achievement in being awarded the Manufacturer Certification by the National Glycohemoglobin Standardization Program (NGSP); for direct HbA1c testing on three of its clinical chemistry analysers; the RX modena, RX imola and RX daytona+.

NGSP is recommended for laboratories conducting diabetes-related clinical trials and is only granted on the basis of 98% accuracy. With the global prevalence of diabetes mellitus increasing rapidly, affecting roughly 8% of the total population, the achievement of this certification emphasises that Randox RX series clinical chemistry analysers correlate with global standards and deliver accurate, reliable and precise results for direct HbA1c testing; helping clinicians make informed decisions for patients with diabetes.

Revealing H-FABP for AKI Testing

A new testing application for the biomarker heart-type fatty acid-binding protein (H-FABP) has been announced by global diagnostic company Randox Laboratories.

Whilst H-FABO is most commonly recognised as an early biomarker of myocardial infarction, the assay’s clinical utility in cardiac surgery-associated acute kidney injury (CSA-AKI) is notable. Studies have shown that patients who developed AKI following cardiac surgery had elevated levels of H-FABP both pre-and post-operatively compared to patients who did not.
